Subcutaneous Tissue Atrophy Misdiagnosed as Deltoid Rupture after Open Rotator Cuff Repair - Case Report -

|
It is a common belief that dimple in the acromial origin of the deltoid may be the clinical sign of the deltoid detachment after the open rotator cuff repair and surgical treatment should be considered. A 56 year female patient with the skin dimple on deltoid origin complained of the poor shoulder function with persistent pain after open rotator cuff repair. She underwent shoulder MRI and sonogram under the impression of the postoperative deltoid detachment. Small size bursal side partial supraspinatus tear and subcutaneous fat tissue atrophy without deltoid detachment were diagnosed. After 2 months of rehabilitation, pain decreased and shoulder function was improved. Because the clinical symptoms originated from improper postoperative rehabilitation were similar with those of deltoid atrophy, it may have misdiagnosed. We suggest that we should definitize the treatment plan by the meticulous physical examination and precise radiologic modality.
